If your University issued documents have been lost, defaced, or destroyed and you wish to obtain a replacement, you can email our Registry team.

Please note: As there can only be one certificate in circulation, in order for us to provide a replacement certificate we would require a statement of declaration from you (wording shown below) to confirm that the original certificate has been lost or destroyed. Please enter this statement into the additional information section.
“I declare that I am requesting a replacement certificate on the grounds that I believe the original certificate has been lost or destroyed. I understand that in the event of finding my original certificate, I will return it to the Health Sciences University immediately”.
